Mullion & Constantine Group Practice

If  you live in our practice area and wish to register with the practice please complete your medical card (if you still have one) and bring it with you to the surgery.  We can register you without your medical card, however,  if you have it to hand we would prefer you to bring it with you. Please note:  The Health Authorities no longer issue medical cards to newly registered patients - should you ever need to know your unique NHS number, please ask at your surgery.

The receptionist will ask you to make an appointment with our Practice Nurse to have a 'New Patient Check' . This involves a 15 to 20 minute appointment where you will be asked a few questions about your present and past health. This is to  make sure your GP has some current background  should you need to be seen before your paper/computer notes arrive  from your previous surgery - for this reason it is important  you attend this appointment.

Please note: we cannot register anyone who lives outside our practice area - if you are in any doubt as to whether you live inside or outside our practice area please telephone us beforehand.
